Have you been diagnosed with atopic dermatitis (eczema) for 1 year or longer?

Have you been diagnosed with chronic rhinosinusitis without nasal polyps for at least 12 weeks?

Do you have access to, or can you obtain documentation of your asthma diagnosis? This can be an after-visit summary from any treating provider or physician or from medical records.

Are you scheduled to have sinus surgery during the study, which may last up to 33 weeks?
