Have you been diagnosed with asthma for at least one year?

Do you have access to, or can you obtain documentation of your asthma diagnosis? This can be an after-visit summary from any treating provider or physician or from medical records.

Do you have any, or have you ever had, of the following medical conditions often associated with a Non-Alcoholic Steatohepatitis (NASH/MASH) or Fatty Liver? (Please select all that apply)

Does one of the following statements apply to you?  [PATIENT NOTE]:  Former smokers are defined as those who have stopped smoking at least 6 months ago.

Do you have a history of any of the following complications of cirrhosis?  - Ascites - Variceal bleeding

Do you have a history of any liver conditions other than NASH/MASH or cirrhosis, such as?  - Hepatitis B  - Alcoholic liver disease  - Liver cancer  - Alpha-1-antitrypsin deficiency  - Wilson's disease  - Liver Transplant or awaiting liver transplant
