l am a current smoker (smoked within the last 6 months)

I am a former heavy smoker (10 pack-years or more)

I am a former light smoker (less than 10 pack-years)

Have you been diagnosed with asthma for at least one year?

Do you have access to, or can you obtain documentation of your asthma diagnosis? This can be an after-visit summary from any treating provider or physician or from medical records.

Will you be willing to sign a medical release form so the clinic staff can request your medical records to verify your asthma diagnosis?

Do you use Albuterol to treat your asthma?

Are you taking 2 or more prescription medications daily to treat your asthma? 

Have you been diagnosed with a lung disease other than asthma? 

Do you receive dialysis treatments for chronic kidney failure?

Are you a current or former smoker? This includes vaping and e-cigarettes. 

Do you live with a chronic viral infection [e.g., Hepatitis B, Hepatitis C]?

If you qualified for this study, would you be willing to complete daily questionnaires about your asthma?   [PATIENT NOTE]:  Questionnaires will be completed on a handheld device provided by the study
